Job Summary
The Community Outreach Team Member – Non-Clinical supports Deborah Heart and Lung Center’s RuralCare Connect program through community engagement, outreach coordination, patient navigation, and health education activities throughout Burlington and Ocean Counties. This position represents Deborah at community events, health screenings, mobile outreach initiatives, and partner locations to increase awareness of available healthcare services and improve access to care for rural and underserved populations.
The Community Outreach Team Member will assist individuals with program information, scheduling assistance, referrals, follow-up coordination, and linkage to Deborah services through collaboration with the RuralCare Connect call center, outreach staff, and clinical teams.
Key Responsibilities
Represent Deborah and RuralCare Connect at community outreach events, screenings, educational programs, and partner locations
Assist with event setup, registration, patient engagement, and community resource coordination
Provide basic health education and program navigation support to community members
Facilitate referrals and follow-up coordination through collaboration with the RuralCare Connect call center and clinical teams
Assist with tracking outreach metrics, participation data, and community engagement activities
Support mobile screening initiatives and outreach operations throughout the service region
Build and maintain positive relationships with community organizations, civic groups, and partner agencies
Ensure accurate documentation and reporting related to outreach activities
Experience
Required: includes community outreach, health education, social services, patient navigation, customer service, or related community-based work. Strong interpersonal, organizational, and communication skills are required.
Schedule Requirements
This is a full-time position that requires flexibility to support community outreach events, health screenings, and program initiatives during daytime, evening, and weekend hours as needed. Travel throughout the service area is required.
HOURS: full-time 80 hours per pay period
This position is grant funded and aligned with a specialized program initiative. Ongoing employment is contingent upon continued program funding and organizational need.
The grant approved annual salary for this position is $62,500.
